Are user input fields accompanied by clear instructions on what the user needs to do to successfully complete the form?
Answer: Never
Score: 0
Notes: I do not see any instructions beyond the form labels.
Do user input fields contain labels in addition to overall instructions mentioned above?
Answer: Sometimes
Score: 1
Notes: The Wave extension reveals many missing form labels - on one search result page, there were 162 listed. Visually, things look fairly straightforward. There is use of placeholder text, but it usually supplements a word describing the box, i.e. Vol. and then the placeholder text reads 52. Some instructions are only available in placeholder text, however. For example, in the Advanced Search, it says Published by in front of the box, but then in the box, the placeholder text reads Enclose publisher's name in quotation marks There are Submit buttons on all forms.
Are descriptions for fields and boxes easy to understand?
Answer: Sometimes
Score: 1
Notes: For the most part, they are straightforward.
Are labels placed above the user input fields and clearly associated with each field?
Answer: Never
Score: 0
Notes: Form labels are always in front of the input fields, on the same line.
Are text cues for coloured form control labels available?
Answer: Sometimes
Score: 1
Notes: The website does not appear to use coloured form control labels. But when I tried to submit a form without filling out all the required fields, the missing fields were highlighted in red. One of the three fields did have an asterisk, but the other two had no other indicators that they were the missing required fields.
Are other visual indicators provided consistently, such as an asterisk for required form fields?
Answer: Sometimes
Score: 1
Notes: A couple of forms - ordering a missed issue, subscribing - don't have an asterisk next to required information - postal codes, state/province
Are user input fields either reversible, checked, or confirmed? One of the three is acceptable.
Answer: Sometimes
Score: 1
Notes: You can go back and change a field that you've already completed. One form I filled out asked me to review the information before I submitted, and also had Go Back options at the bottom of the screen. The only instance of data being automatically being check for accuracy was to see if all the mandatory fields had been filled out.
Are alert messages clearly visible to the user, either at the top of the form field, inline, or through a dialog box?
Answer: Always
Score: 2
Notes:
Do alert messages clearly indicate the field in which the error has occurred and a description of the error and the fix?
Answer: Always
Score: 2
Notes:
Is the user able to easily access the user input field to correct the error?
Answer: Sometimes
Score: 1
Notes: Two fields - State/Province and Zip Code/Postal Code - are required fields but do not have an asterisk. When the form is submitted, the error message comes up, but on the next page where the user is supposed to input their billing information. There is no option to add this information. It is possible to use the back browser to return to the form and add this information, but it is not intuitive.
Is the user able to resubmit the form and re-validate their submission?
Answer: Always
Score: 2
Notes:
Do error reports follow a logical reading and navigation order?
Answer: Always
Score: 2
Notes: No error reports.
When a user spells a word incorrectly, does the search function offer spelling suggestions or synonyms?

Answer: Never
Score: 0
Notes: I wasn't able to open any of the menus or submenus using keyboard navigation. When you tab from the Choice Reviews logo at the top of the page, you are able to tab to different search options in the dropdown menu, but there is nothing to indicate that that is what you are doing. The keyboard focus just disappears and you can see as you tab that the URL is changing by a popup at the bottom left of the screen with a black bar containing a URL.

Are you able to tab through a page using keyboard navigation without the keyboard focus moving away from the control?
Answer: Sometimes
Score: 1
Notes: When you first start keyboard navigation on a page, I had to hit tab 37 or 38 times before the focus reappeared. It appears when you first hit tab at the Choice Reviews logo, and then you're tabbing through a bunch of invisible options until the focus finally reappears on Reviews, the next logical focus point. It seems to tab through menu options from the dropdown menus on the screen, but I only could determine this by looking at the black popup at the bottom of the screen with the search navigation, and then comparing it to the URL indicated when hovering over the searches in the menu dropdown.
