Are user input fields accompanied by clear instructions on what the user needs to do to successfully complete the form?

Notes: In the 'Body Weight Calculator' and the 'Renal Function Calculator', the weight and height input fields are labeled as 'spinbuttons' and that is read out loud with NVDA. Otherwise most 'pill identifier' fields are read out loud with clear instructions. A few of the fields had the grey placeholder text, which was not read out loud. Hover Speech on Read&Write also ignored the greyed out print.
Do user input fields contain labels in addition to overall instructions mentioned above?
